Job description:

We're looking for a Full-Time Lead Physical Therapist to become the clinical leader of an outpatient home therapy program.

You'll provide meaningful one-on-one care while taking ownership of the therapy program, building relationships with patients and families, collaborating with referring providers and care teams, and helping create a standard of rehabilitation that truly improves lives.

Responsibilities:

• Provide high-quality, one-on-one physical therapy to patients in their homes.

• Serve as the primary therapy leader.

• Build strong relationships with patients, families, caregivers, and referring providers.

• Help develop fall-prevention, mobility, wellness, and functional independence initiatives.

• Support and mentor other therapy team members as the program grows.

• Help establish clinical standards and best practices.

• Participate in interdisciplinary care planning.

• Serve as a trusted rehabilitation resource for patients and their support networks.

• Help grow a therapy program focused on exceptional outcomes rather than volume.
